HealthWorks invites our fitness members to participate in our annual Multiphasic Blood Screening on March 23, 2012 7:30 to 10:00am

Multiphasic $30.00—PSA (men only) $10.00—Thyroid $15.00

Humana/PEIA insurance can be billed.  You must have your insurance card with you so we can verify your coverage and bill the cost of your lab work to Humana/PEIA.  With the insurance card you will not have any out of poscket cost for your lab work.  The cost for all others will be collected at the time of service.
